As an Independent Distributor of MAYI Life Bangladesh Ltd, I agree to the following terms and conditions.
১। আমিআইনঅনুযায়ীভাবে ১৮ বছরেরবেশিবয়সীএকজনব্যক্তি, মায়ইলাইফবাংলাদেশলিমিটেডএরনিময়মেনেনিজেকেএকজনস্বাধীনপরিবেশকহিসেবেআবেদনকরছি।
1.I am a person above 18 years of legal age, I am applying as an Independent Distributor as per the rules of MAYI Life Bangladesh Limited.
২।স্বেচ্ছায়ওস্বজ্ঞানেঘোষণাকরছিযেআমিমায়ইলাইফবাংলাদেশলিমিটেডএ কর্মরতকোনকর্মচারীবাপরিচালকবাঅংশীদারনই।
2. I herebyvoluntarily and consciously declare that, I am not an employee or director of partner working in MAYI Life Bangladesh Limited.
৩. আমিমায়ইলাইফবাংলাদেশলিমিটেডএরবাণিজ্যিকনামবাট্রেডমার্কবাপরিষেবাচিহ্নঅনুমতিব্যাতিতব্যবহারকরবনা। মায়ইলাইফবাংলাদেশলিমিটেডএরলিখিতসম্মতিছাড়াকোনোধরনেরবিজ্ঞাপন ও প্রচারকরবনা।
3. I shal not use the Trade Name or Trademark or service mark/Logo of MAYI Life Bangladesh Limited without permission. I will not publish any advertisement without the written consent of MAYI Life Bangladesh Limited.
৪। আমিকোম্পানি ও কোম্পানিরস্বাধীনপরিবেশকগণেরবিরুদ্ধেআক্রমনাত্বকবা অ-আইনানুগএবংউপদ্রবসৃষ্টিকরেএমনকোনোকথাবলবোনাবালিখিতকোনবিবৃতিদেবনা।
4. I will not speak or make any written statement that is offensive or illegal and creates nuisance against the Company and its Independent Distributors.
৫। আমিমায়ইলাইফবাংলাদেশলিমিটেডএরলিখিতসম্মতিছাড়াপণ্যেরবিষয়বস্তুরকোনোরূপপরিবর্তনকরবনা। মায়ইলাইফবাংলাদেশলিমিটেডদ্বারানির্দেশিতবিক্রয়প্রচারকার্যক্রমছাড়াকোনোপ্রকারলিফলেটবানোটিশবিতরণকরবনা।
5. I will not make any changes to the content of the product without the written consent of MAYI Life Bangladesh Limited. No leaflets of notices will be distributed except for sales promotion activities directed by MAYI Life Bangladesh Limited.
৬। আমিমায়ইলাইফবাংলাদেশলিমিটেডএরপন্যগুলরবর্ণনাবিস্তারিতএবংসততারসঙ্গেতুলেধরব। আমিমায়ইলাইফবাংলাদেশলিমিটেডএরপণ্যসম্পর্কেকোনওমিথ্যাবাপ্রতারণামূলকদাবিকরবনা।
6. I will describe MAYI Life Bangladesh Limited products in details and honestly. I won’t do any false or fraudulent claims regarding MAYI Life Bangladesh Limited products.
৭। মায়ইলাইফবাংলাদেশলিমিটেডএরপণ্যসমুহেরদ্বারাকোনপ্রকারডাক্তারীপরামর্শপ্রদানকরেনা। পণ্যগুলোঔষধনয়, উৎকৃষ্টমানসম্পন্নখাদ্যপণ্য,ত্বকেরজন্যউপকারীস্বাস্থ্যপণ্যসমূহ।
7. MAYI Life Bangladesh Limited does not provide any medical advice by way of consignment. The products are not medicines but high-quality food products, skin-friendly and health-supporting products.
৮। আমিগ্রাহকদেরকাছেপণ্যেরব্যাখ্যাদেয়ারসময়মায়ইলাইফপণ্যেরলেবেলবাম্যানুয়ালঅনুসরণকরব। মায়ইলাইফএরপণ্যসম্পর্কেঅতিরঞ্জিতবাঅবাস্তবএমনকিঅপ্রাসঙ্গিকযেকোনোতথ্যবর্ণনাথেকেবিরতথাকব।
8. I will follow MAYI Life’s product labels or manuals when explaining the product to customers. I will refrain from describing any Exaggerated or unrealistic about MAYI Life’s products information that it not even relevant.
৯। আমিএকমতপোষণকরছিযে,মায়ইলাইফবাংলাদেশলিমিটেডএরপরিবেশকহিসেবেআমিকোনপরিবেশকবাভোক্তাগণেরচাকুরিরপ্রস্তাবপ্রদানকরাথেকেবিরতথাকব। শুধুমাত্রস্বাধীনপরিবেশক ও ভোক্তাহিসেবেকোম্পানিরসাথেযুক্তহয়েপণ্যসমুহেরক্রয়-বিক্রয়করবো।
9. I agree that as a distributor of MAYI Life Bangladesh Limited, I shall refrain from offering any distributor or consumer employment. Only as an independent distributor and consumer will associate with the company to buy and sell the goods.
১০। আমিমায়ইলাইফবাংলাদেশলিমিটেডথেকেপ্রাপ্তসমস্তউপার্জনেরতথ্যজাতীয়রাজস্ববোর্ডেরকাছেযথাযথভাবেপ্রকাশকরবএবংআমারউপরআরোপিতসমস্তকরপরিশোধকরব।
10. I will pay duly disclose all earnings information received from MAYI Life Bangladesh Limited to the National Board of Revenue (NBR) and will pay all taxes imposed on me.
১১। আমিযদিনতুনকোনোযোগদানকারীকেকোম্পানিরসঙ্গেপরিচয়করিয়েদেইঅথবাস্পন্সরকরিসেক্ষেত্রেযদিনতুনযোগদানকারীযোগদানের ০৭(সাত) দিনেরমধ্যেকোনোপণ্যফেরৎ দিতেচায়আমি ০৭(সাত) দিনেরমধ্যেসেইপন্যফেরতদেওয়ারজন্যদায়ীথাকব।
11. If I introduce or sponsor a new enrollment to the company, if he/she returns any product within 07(seven) days of enrollment, I will be responsible for returning the product within 07(seven) days.
১২। আমিস্বজ্ঞানেমতপোষণকরেঘোষণাকরছিযে, আমিযদিমায়ইলাইফবাংলাদেশলিমিটেডএরসাথেবিরোধসৃষ্টিকরতেপারেএমনকোনোআচরণেজড়িতথাকিবাআমারকারণেযদিমায়ইলাইফবাংলাদেশলিমিটেডঅন্যেরসঙ্গেবিরোধেরসুযোগতৈরিহয়বাআমিকোনোসরকারিআইন ও বিধানলঙ্ঘনকরিবামায়ইলাইফবাংলাদেশলিমিটেডএরনীতিরসঙ্গেসাংঘর্ষিকএমনকাজকরিতাহলেকোম্পানিযেকোনসময়কারণদর্শানোরনোটিশদেওয়া ও কোম্পানিরসিদ্বান্তেপরিবেশিকমর্যাদাবাতিলসহযথাযথআইনানুগব্যবস্থাগ্রহণকরতেপারবে।
12. I declare knowingly that if I violate or fail to uphold the agreement made with MAYI Life Bangladesh Limited or do not follow the terms and conditions stated in the agreement or engage in any conduct that may create dispute for my activity with MAYI Life Bangladesh Limited or if there is a chance of dispute between MAYI Life Bangladesh Limited and others due to this, or if I violate any government law and regulations or act in conflict with the policy of MAYI Life Bangladesh Limited, then the company may take appropriate legal action against me at any time including giving a show cause notice and canceling the distributor status at the company’s decision.
১৩। প্রত্যেকপরিবেশককেপ্রতিবছরনূন্যতম ০১ টিমায়ইলাইফপণ্যক্রয়করতেহবে। কেউযদিনূন্যতম ০১ টিমায়ইলাইফপণ্যক্রয়করতেব্যর্থহয় (যোগদানেরতারিখহতে ০১ বছর) তাহলেতারপরিবেশকমর্যাদাহারাবেন।
13. Each distributor must purchase a minimum of 01 MAYI Life product in a year. If anyone fails to purchase at least 01 product in a year (From enrollment date to one year) then he/she loses Distributorship status.
আবেদনকারী/পরিবেশক এবং জামিনদারের যৌথ ঘোষণা: আমি মায়ি লাইফ বাংলাদেশ লিমিটেডের সমস্ত নিয়ম ও পদ্ধতি পড়েছি এবং তাতে সম্মত এবং ভবিষ্যতে কোম্পানি কর্তৃক প্রযোজ্য যেকোনো পরিবর্তন, সংশোধন বা সংযোজন-বিয়োজনের ক্ষেত্রে সমস্ত পৃষ্ঠায় লেখা সমস্ত পদ্ধতি ও চুক্তি এবং সমস্ত নিয়ম মেনে চলার অঙ্গীকার করছি।
Joint Declaration of Applicant / Distributor and Sponsor: I have read and accepted all the rules and procedures by MAYI Life Bangladesh Limited and undertake to fully comply with all the procedures and agreements and written rules on all pages of any future changes or amendments of additions applicable by the company.
